Care Management Services
What is a Care Manager?
A professional care manager is an individual with specialized training in the physical, emotional and psychosocial challenges that older adults and persons with special needs face. Our care manager have an in-depth knowledge of supportive services to meet those challenges.
The goal of a care manager is to assist older adults and persons with disabilities in attaining their maximum functional potential and highest quality of life. Your care manager will work with you to identify and coordinate your specific care needs in any situation.
At Herb Thomas and Associates, our professional care managers are focused on providing client-centered services. They serve as advocates and guides through challenging and often overwhelming life events. We assist individuals in maintaining their preferred level of independence, provide information to help make informed life decisions, and serve as advocates for health and well-being.
Care management services support individuals and families; older adults; persons with special needs; and individuals with chronic and acute illnesses. Our personal and attentive services are tailored to meet the needs of each unique individual we serve.
